MnCHOICES is a comprehensive web-based application that integrates assessment and support planning for Minnesotans who need long-term services and supports. MnCHOICES is for people of all ages who have any type of disability or need for long-term services and supports. MnCHOICES embraces a person-centered approach to ensure services meet each person's strengths, goals, preferences and assessed needs. 

To determine if you or your family member can participate in one of the home and community based waiver programs we will ask the following questions:
- Are the services necessary to ensure the recipients health, welfare and safety?
- Will the services address skill development and/or skill maintenance?
- Is the service covered by any other funding source, such as Medical Assistance, private pay health insurance, Medicare, education or vocational rehab service?
- Have all options been assessed and does this option meet the individual desires, needs and preferences of the person?
- Is the cost of the service considered reasonable and customary?

Long Term Care Services via MN Waiver Programs Offered in Houston County:

This program helps Minnesotans age 65 and older with low income and few assets stay in their homes and communities instead of going into a nursing home.
A program for people who have a brain injury from an accident or illness and need care from a specialized nursing facility that has cognitive and behavioral supports for brain injuries or neurobehavioral hospital level of care.
This program is for people with disabilities that require the level of care provided in a hospital.
The Community Access for Disability Inclusion (CADI) Waiver provides funding for home and community-based services for children and adults, who would otherwise require the level of care provided in a nursing facility.
A program for people with developmental disabilities or related conditions who need the level of care provided in an intermediate care facility for people with developmental disabilities.
This program serves residents older than 65 who require the level of care provided in a nursing facility.
Community First Services and Supports is a program for people who have personal care needs who are eligible for MN healthcare programs. CFSS allows people greater independence in their homes and communities. 
Family Support Grant (FSG) program provides state cash grants to families of children with certified disabilities. The goal is to prevent or delay out-of-home placement of children with disabilities and promote family health and social well-being by providing access to family-centered services and supports.
